ITB pain, also known as iliotibial band syndrome (ITBS), is a common cause of pain on the outside of the knee, particularly in runners and cyclists. It often develops gradually and can become particularly noticeable when running downhill, increasing mileage or returning to training after a period of reduced activity.
Although it is commonly called "IT band friction syndrome", current understanding suggests that ITB-related pain is more complex than simply the IT band repeatedly rubbing over the outside of the knee.

What is the IT band?
The iliotibial band (ITB) is a strong band of connective tissue that runs along the outside of the thigh.
It extends from the area around the hip towards the outside of the knee, where it attaches to the upper part of the tibia.
The ITB helps transmit forces between the hip and knee and contributes to stability during activities such as:

What is ITB syndrome?

Some people also experience discomfort further up the outside of the thigh or around the outside of the hip.
The location of pain is useful, but several other conditions can cause lateral knee pain. A proper assessment is therefore important before assuming that the ITB is responsible.
ITB Pain in Runners
ITB syndrome is particularly common in runners.
It may develop after:

A common pattern is being able to run for a certain distance before the pain gradually appears.

Cyclists can also develop pain around the outside of the knee.
Cycling involves thousands of repetitive knee movements, meaning relatively small changes in training load or bike position can produce a large increase in cumulative load.

Hip weakness is often discussed as a cause of ITB syndrome, but the relationship is more complicated.
Reduced hip strength or endurance can contribute to altered lower-limb mechanics during running and other single-leg activities.
However, ITB pain is not simply caused by having "weak glutes".
Your physiotherapist will consider strength alongside training load, movement patterns, running demands and other individual factors.
ITB Pain Treatment
The main focus of treatment is usually load management and progressive strengthening.
Rather than simply stretching the IT band or stopping all activity, rehabilitation aims to improve your ability to tolerate the demands of running, cycling or sport.

Should I Stop Running With ITB Pain?
Not necessarily.
Some runners can continue running with modifications, particularly if symptoms remain manageable and settle appropriately afterwards.

If pain is severe, worsening rapidly or significantly changing your running pattern, a greater reduction in load may be appropriate.
Does Stretching the IT Band Help?
The IT band is a strong connective tissue structure and cannot be meaningfully "stretched" in the same way as a muscle.
Aggressive ITB stretching is therefore not usually the main focus of rehabilitation.
Instead, treatment generally focuses on managing the provoking load and improving strength and movement capacity.
Soft-tissue techniques may sometimes provide short-term symptom relief but should not replace progressive rehabilitation.
How Long Does ITB Syndrome Take to Recover?
Recovery varies between individuals.
Some people improve within several weeks after modifying their training and beginning appropriate rehabilitation.
Long-standing ITB pain may take longer, particularly if the underlying training load or contributing factors have not been addressed.
The goal is to gradually increase your ability to tolerate the activity that previously caused symptoms.
Can ITB Syndrome Cause Permanent Damage?
ITB syndrome generally does not cause permanent damage to the knee.
However, continuing to repeatedly aggravate the symptoms without addressing the underlying factors can result in persistent pain and repeated flare-ups.
A structured rehabilitation programme can help restore your ability to run or exercise normally.

Not all pain on the outside of the knee is caused by the ITB.

Can I run with ITB pain?
Often, yes, depending on the severity of your symptoms.
You may be able to continue running at a reduced distance or intensity while working through rehabilitation. If running causes significant or progressively worsening pain, your training load should be reduced further.
Should I stretch my IT band?

Can cyclists get ITB syndrome?

Training volume, resistance, bike setup and individual movement patterns may all be relevant.
How long does ITB syndrome take to heal?
Many cases improve over several weeks with appropriate rehabilitation, although persistent cases can take longer.
The timeline depends on how long you have had symptoms, your training demands and how quickly your capacity can be rebuilt.
Do I need an MRI for ITB syndrome?
Usually not.
ITB syndrome can often be diagnosed through a clinical assessment. MRI may be considered if symptoms are atypical, persistent or if another condition needs to be investigated.
Can ITB syndrome come back?
Yes. Symptoms can recur if running or training load is increased faster than your capacity can tolerate.
Maintaining appropriate strength and gradually progressing high-demand activities can help reduce the risk of recurrence.
When should I see a physiotherapist for ITB pain?
Consider an assessment if your pain is preventing you from running or cycling, keeps returning when you increase training, or is not improving despite modifying your activity.
